From the Patient Information, or any clinical
screen, clicking the Forms button brings up a screen which lets you select
one of the forms to be filled out. The Paperless Office for Chiropractic®
will automatically fill in the form with information in the clinical system
saving time and data entry steps.
The system includes well over a hundred
pre-programmed forms, but you can have CSC add your own for a nominal fee
per form.

Forms may also be saved for future reference
in the patient chart. Clicking on the ‘Previous’ button displays a list of
saved forms for the patient and double clicking will display the form.

Previewing, printing, emailing and faxing are
available from this screen once the form has been filled out. To print the
form, simply press the ‘Print’ button. (See next page)

Treatment plans can be filled out and
maintained quickly using this feature.

Electronic signatures are secured and require
password authorization.
The form image is generated, completely filled in as though it were typed,
complete with logos or any other format from the original blank form.
